Distal Biceps Tendon Rupture

If you suspect a distal biceps rupture, seek evaluation within 1–2 weeks. Delayed repair (beyond 4–6 weeks) is significantly more difficult, has higher complication rates, and may require tendon grafting. Early repair produces the best outcomes.
What Is a Distal Biceps Rupture?
The distal biceps tendon connects the biceps muscle to the radius bone at the elbow. When this tendon ruptures, the biceps muscle retracts upward in the arm, creating a visible deformity and significant loss of function — particularly supination (turning the palm up) and elbow flexion strength.
This injury most commonly occurs in men aged 40–60 during heavy lifting — typically when an unexpected eccentric load is applied (catching a heavy falling object, lifting something heavier than expected).
How Does It Happen?
The classic mechanism is an eccentric load on the flexed elbow:
- Catching a heavy falling object
- Lifting furniture or heavy equipment
- Sudden unexpected load during a biceps curl or deadlift
- Pulling a stuck lawnmower cord
The patient typically reports:
- A sudden "pop" at the elbow crease
- Immediate pain and weakness
- Bruising that develops over 24–48 hours (often extensive, tracking down the forearm)
- A visible "bunching up" of the biceps muscle in the upper arm
What You'll Notice

What You Lose Without Repair
If left unrepaired, a distal biceps rupture results in:
- 40–50% loss of supination strength (turning palm up — critical for using a screwdriver, turning a doorknob, opening a jar)
- 30% loss of elbow flexion strength
- Significant loss of supination endurance (fatigue with repetitive tasks)
- Permanent cosmetic deformity ("Popeye arm")
For active patients, manual laborers, or anyone who values arm strength and function, surgical repair is strongly recommended.
Diagnosis
- Physical examination: Hook test, squeeze test, visible deformity — often diagnostic
- MRI without contrast: Confirms the diagnosis and shows the degree of tendon retraction. We obtain MRI prior to surgery to plan the approach.
Surgical Repair
Timing Is Critical

Postoperative Course

Key points:
- The tendon takes 12 weeks to heal to bone — this is biology and cannot be rushed
- 6 months for full rehabilitation and return to pre-injury strength
- We often immobilize initially (though not all surgeons feel this is required)
- No heavy lifting until 12 weeks minimum
Complication Rates
Distal biceps repair is a successful procedure but carries a 15–25% overall complication rate [2][3], most of which are minor and self-resolving:

Important Context
The major complication rate (requiring reoperation or causing permanent deficit) is approximately 4.5–7.5% [4]. The majority of complications are minor nerve palsies that resolve spontaneously within weeks to months.
Acute vs. Delayed Repair: What the Literature Shows

Bottom line: Functional outcomes can still be good with delayed repair, but the complication rate nearly doubles and the surgery is more complex. This is why we urge evaluation within 1–2 weeks of injury.
Common Questions
How do I know if my biceps is ruptured?
A sudden pop at the elbow during heavy lifting, followed by bruising, weakness in supination, and a visible "Popeye" deformity is highly suggestive. The hook test (inability to feel the tendon at the elbow crease) is nearly diagnostic.
Can I just leave it alone?
You can, but you will lose approximately 40–50% of supination strength and 30% of flexion strength permanently. For sedentary patients or those with low physical demands, nonoperative management is an option. For active patients, repair is strongly recommended.
How long until I can lift again?
12 weeks for light lifting (the tendon must heal to bone). 6 months for full return to heavy lifting and sport.
What if it's been more than a month since my injury?
Repair may still be possible, but it becomes more complex. We may need to use a tendon graft (allograft). The sooner you are evaluated, the better.
I don't do LHB repairs — is that different?
Yes. A long head of the biceps (LHB) rupture at the shoulder is a completely different injury. LHB ruptures are associated with rotator cuff tendinopathy and are often curative of shoulder pain. They rarely need repair. I do not perform LHB repairs (tenodesis), but can refer if indicated. → Rotator Cuff Tendinopathy
